We will simplify the process and fast-track your enrollment in the Self Determination Program (SDP). We are here to serve you.

Book a Phone Consultation
We will have a personal meeting with you, where we will go over your concerns and timelines.

Prepare your Spending Plan
Your Regional Center has approved your spending plan.

Submit your documents
We have all of your important documents completed and signed

Approving by the regional center
All of your documents, including the spending plan, must be submitted to your regional center for a final review.

Welcome to Empriza FMS
We will send you a welcome package that includes your projected monthly spending budgets. This welcome package may be sent digitally or by mail, or both. We can’t wait to help support you and be by your side.
